MISSED VISITS

Please contact office via email or call if patient has missed 2-3 visits in a row.

DISCHARGES

·       Regular Discharges - DC per auth and POC. Oasis DC completed by therapy if they are last discipline with pt. Please contact agency prior to DC to determine what type of DC is required.

Transfer completed by last discipline to see pt prior to hospitalization

Resumption of Care and Recert
