SOC Packet (copy of insurance card is required, see below for full list of SOC requirements). SOC packets now sent to therapist email via Docusign. All SOC and DC paperwork done electronically via Docusign.
Staffing/Scheduling Questions: Call Kelly Dobson or request assistance from the ATS Ops Team
Additional Authorization Process (create a comm note in Alpine Kinnser with the Additional Auth Request template & email cory@alpinetherapyservices.com - title of the email “Additional Authorization Request” submitted for patient (first name, last initial and date)
Insurance Information:
Look in SOLI to view insurance type and authorized number of visits at SOC or eval. Kaiser will likely not approve additional authorization (rarely), but other insurances typically will approve additional authorization. If additional authorization is needed, please input a comm note in Alpine Kinnser and notify the ATS Admin Team via instructions above, who will email the Alliant auth team for assistance and approval. Wait to see the patient until authorization has been confirmed.
Medicare - Please contact Kristin Lang at (720) 620-5568 for approval of frequency for PDGM purposes before obtaining a VO.

Required SOC Documents in Alliant Kinnser:
1) SOC signed paperwork uploaded under the MISC tab in Kinnser (Admission consent, emergency preparedness, agency disclosure notice, ABN, LACE index, mini COG, insurance card)
2) Case conference
3) Physician order
4) visit verify signature (either signature uploaded from SOLI or signature in Kinnser)
5) sign off on 485 (not due until Alliant has reviewed your SOC)
