Input VO on the evaluation, unless greater than 24hrs in which you will need a separate MD order

Change frequency to make up the approved visit
3 consecutive missed visits, discuss compliance as an issue with agency before DC
DISCHARGES

Regular Discharges
The last one in does the DC OASIS.
Have the patient sign the NOMNC if appropriate
Transfers
Make sure to find out 1) what day the patient went to the hospital 2) what hospital they are located and 3) if they were admitted or under observation
o Observation does not require a transfer; Admittance requires a Transfer OASIS
· Notify the ATS Task Force & the primary contact for each company with the above information
· Complete the Transfer OASIS if necessary; ask the agency before completing
· Complete Physician Order (click File – New – Order (Transfer/Hospitalization Order)
· Submit the patient in Soli as ‘hospitalized.’
Resumption of Care
Communicate with the main office.
