She is in charge of the full 60 day episode including utilization, optimization and compliance with documentation. Missed visits also need to be called to PCM.

Things to know:
All SOC/ROCs and add on evals require a call in to Sabrina for a driveway call. Click here to see what this looks like. This note can’t be completed by the clinician they must call it in.
Visits that can’t be completed and need to be moved must be returned in one of two ways: Reschedule – meaning that same clinician is going to see the patient on a different day and they plot for that day or: re-assign which means that clinician is unable to see the patient and it needs to go to another clinician. Re-assign need to have a scheduler notification coordination note attached.
Narrative Notes are no longer required. Write all details in the Interventions/Goals section and Therapy Assess/Plan, which will populate a SOAP note to be viewed in Medical Records.
If a patient goes to the hospital, input a ‘HH pending transfer to facility’ note in the tablet ASAP.
For quick assistance in the tablet, input a ‘clinical support specialist review asap’ to push it quickly
***Please note: Sync, refresh and renew your tablet once a week at minimum to stay up to date on all changes within PointCare!!!

PointCare Tips as of August 2022
-Care 365- Accent Care is adding a team in Texas that will assist clinicians with after hours and weekend phone calls and assist with visit; this information will be provided through the Accent Care main number.
-Documentation change as of 2022 - Accent Care took away redundant questions from the SOC
-Clinicians need to choose only 4 pathways now: this is under actions>> pathways:
Choose additional visits need to be provided pathway (there is an option to chose eval only if appropriate)>> save
Choose focus of care>>fill in the blanks
Choose appropriate (low/moderate/high) risk for hospitalization and change narrative as appropriate
SOC statement pathway
Click save when complete the above 4 pathways. Validation failure may show up. It is ok to continue as long as the 4 pathways were chosen and completed
They basically want 90% of documentation be completed at home
After leaving patient’s home, call agency to report SOC and POC.
If don’t get VO within reasonable time>>fill agency form and request to fax.
